How to Enroll

Follow these easy steps to log in to our benefits enrollment website.

How to Enroll

1. Select New User Registration.
2. Enter your First Name, Last Name, Company Identifier, Last 4 Digits of your SSN, and Date of Birth. Your Company Identifier is “COMPANY NAME.”
3. Update your personal information and add your dependents.4. Complete the enrollment for each – Medical, Dental, Vision, Basic Life, and AD&D.
4. Remember to specify your beneficiaries for your Life insurance plan.

Note for Dependents

In order to add dependents for medical, dental, vision, or supplemental life insurance coverage, you are required to submit dependent verification documents and receive approval from HR before coverage can begin. See Dependent Documentation Required for details about what qualifies as documentation.

To Enroll Family Members

If you’re enrolling a family member for medical, dental, or vision coverage for the first time, you’ll need to enter that person’s name, relationship to you, gender, a valid Social Security number, date of birth, and address (if it’s different from yours).
